Another recurring issue is the absence of risk-based justification when invalidating or confirming OOS results. EMA inspectors routinely find that decisions to invalidate OOS data are based on subjective judgment—“analyst error” or “sample handling anomaly”—without supporting evidence from instrument logs, calibration records, or validation data. Incomplete or contradictory records trigger suspicion of retrospective rationalization.
